- A statement by you that you have a good faith belief that the disputed use is not authorised by the copyright owner, its agent, or the law.
- A statement by you, made under penalty of perjury, that the above information in your notice is accurate and that you are the copyright owner or authorised to act on the copyright owner's behalf.

3. Counter-Notification
If you believe that your content was removed or disabled as a result of mistake or misidentification, you may submit a counter-notification to our Copyright Agent. Your counter-notification must include:
- Your physical or electronic signature.
- Identification of the material that has been removed or disabled and the location where the material appeared before it was removed or disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Your name, address, telephone number, and email address, and a statement that you consent to the jurisdiction of the federal court in your district (or the district where your address is located) and that you will accept service of process from the person who provided the original DMCA notification or an agent of such person.
4. Repeat Infringers
QuerySquish reserves the right to terminate the accounts of users who are determined to be repeat infringers under the DMCA or other applicable laws.
5. Limitations of Liability
QuerySquish is not responsible for any content uploaded by users. We act as a search engine and platform for user-generated content and comply with the DMCA to address claims of copyright infringement.
